Yoh Health Care is seeking a Registered Nurse that is experienced in the transition of care of patients in a managed care setting.

Job Requirements:

Candidate must be able to effectively manage and coordinate the care of patients with the Health plans, to assure the right level of care is provided and that all the patients’ medical, environmental and psychosocial needs are met, during their transition.
Registered Nurse in state of CA
Previous Managed Care experience at least 2 years recent
Experience in transition of care within managed care environment
HIPPA knowledge
